Baby of Vaishnavi Twin 2 is a 49-day-old male baby admitted to the NICU of Rainbow Children’s Hospital. The baby was born as extremely Preterm (26+1 weeks) with Extreme Low Birth Weight (850 grams). The baby was diagnosed as Preterm, Extreme Low Birth Weight, Respiratory Distress, HMD, Culture Positive Sepsis, Apnea of Prematurity, Anemia of Prematurity, Unconjugated Hyperbilirubinemia hS PDA, Hypernatremia of prematurity. The baby was treated with Mechanical Ventilator, surfactant, IV fluids, IV antibiotics, and Inj. Caffeine, IV Paracetamol, LRBC transfusions, and incubator care. The baby was extubated to CPAP support and later weaned off to room air. At 41 days of life, the baby had acute deterioration in the form of bradycardia and de-saturation for which he was intubated and ventilated. In view of the high-pressure requirement, the baby was shifted to high-frequency oscillatory support. At present baby is on mechanical ventilation, IV fluids, and NG tube feeding. In view of prematurity with low birth weight and with the above condition, the baby may require further NICU for 2-3 weeks.
