Baby of Vaishnavi Twin 1 is a 49-day-old male baby admitted to the NICU of Rainbow Children’s Hospital. The baby was born as extremely Preterm (26+1 weeks) with Extreme Low Birth Weight (700 grams). The baby was diagnosed as Preterm, Extreme Low Birth Weight, Respiratory Distress, HMD, Culture Positive Sepsis, Apnea of Prematurity, Anemia of Prematurity, Unconjugated Hyperbilirubinemia hS PDA, Hypernatremia of prematurity. The baby was treated with Mechanical Ventilator, surfactant, IV fluids, IV antibiotics, and Inj. Caffeine, IV Paracetamol, LRBC transfusions, and incubator care. The baby was extubated to CPAP support. In view of the persistent requirement for respiratory support and persistent hemodynamically significant PDA, a device closer was done. At present baby is on CPAP support, IV fluids, and NG tube feeding. In view of prematurity with low birth weight and with the above condition, the baby may require further NICU for 2-3 weeks
